Completing the identity verification form

If you were selected for verification by the Department of Education to confirm information you provided on your Free Application for Federal Student Aid, the FAFSA, please follow these instructions:
  
If you are unable to appear in person, you do not need to contact us. 

Please do this: 
Once you are logged in to StudentForms, review the drop-down menu and choose the “I am unable to appear in person at my school to verify my identity” option. This will prompt you to download and print the form:

Take the printed form to a Notary Public. The Notary Public will need a valid, unexpired, government-issued photo ID: a U.S. passport, a driver's license, or other state-issued ID. 

You may then mail the notarized document with a copy of the government-issued photo ID used to the address listed on the form:
